LOLER Report of Thorough Examination Template

What the worksheet covers

The HSE explains that a thorough examination report has no required visual format, provided it includes the 11 particulars in LOLER Schedule 1. The worksheet groups those particulars into practical sections:

  1. Employer and examination premises
  2. Equipment identity, manufacture date where known and safe working load
  3. Previous examination date and the reason for this examination
  4. Dangerous and potentially dangerous defects, required action and timescales
  5. Next examination date, test particulars and examination date
  6. Report author, qualifications, authentication and report date

This is an educational blank form. A competent person must decide the examination scope, complete the findings, and authenticate the actual report. Downloading it does not create a valid examination record.
